STEP 1A: PRINT THE PETITION

STEP 1B: SIGN THE PETITION (Frederick City Voters Only)
Print clearly. Complete every space. Your signature should match your voter registration as much as possible.
Double check that every blank is completed. First name, middle name, last name, birth date, date of signature, street number, street name, apartment number if applicable, and zip code.
STEP 2: COMPLETE THE CIRCULATOR’S AFFIDAVIT
The person collecting signatures must complete the Circulator’s Affidavit at the bottom of the page.
The circulator’s affidavit is required for petition signatures to be considered valid. Complete every space printing clearly. Include name, residence address, city, state, zip, and telephone. The circulator must sign and date the affidavit. NOTE: You can be a signer and circulator.
